John Wayne Cancer Foundation is holding a "Block the Blaze" hat design contest! To submit a design for the contest, please follow the directions below. You must be a Junior Lifeguard in the state of California to submit artwork!
- Print out this template and create your design!
- Make something you would be proud to wear and shows what Block The Blaze, John Wayne andJunior Lifeguards mean to you!
- The design must include the words John Wayne Cancer Foundation
- Please email your design to lauren@johnwayne.org, or send back to John Wayne Cancer Foundation at: 210 62nd St. Newport Beach, CA 92663
- Be sure to include your first & last name, phone number, email address and name of your Junior Lifeguard agency.
- Submissions deadline is: Monday, August 12, 2019
